The End of Compounded GLP-1 Medications

If you’ve been using compounded versions of semaglutide (Ozempic, Wegovy) or tirzepatide (Mounjaro, Zepbound), the changes that we’ve been discussing for the last year are final here. Due to patent enforcement by Eli Lilly and Novo Nordisk, compounded semaglutide is officially banned from 503A pharmacies as of April 22, 2024, and from 503B pharmacies by May 22, 2024. For tirzepatide, the ban has already taken effect—February 18, 2024, for 503A pharmacies and March 19, 2024, for 503B pharmacies.

Without compounded versions, patients now face increased costs and limited access. Some may turn to black-market sources, but this poses serious safety risks. These medications must be sourced from legitimate, regulated pharmacies to ensure they are effective and safe.

Microdosing Ozempic: A Growing Trend?

As the cost of GLP-1 medications continues to rise, some people are experimenting with microdosing Ozempic—taking doses well below the standard starting level. Instead of the typical 2.5 mg starting dose for Mounjaro/Zepbound, individuals are using as little as 0.5 mg or 1 mg.

Microdosing may help reduce food noise, control cravings, and maintain some benefits of GLP-1 medications at a fraction of the cost. It’s also being explored for its potential role in reducing alcohol consumption and even lowering the risk of dementia.
